The Care Facilitator is responsible for the provision of quality care, learning opportunities, and meaningful engagement for clients to support their life skills development.
Critical Work Functions
Quality Care and Optimal Client Engagement
Planning:
Data Collection: Monitor, observe and record clients’ progress to provide data for development of learning objectives, outcomes, and/or relevant plans
Activity Preparation: Prepare materials and environments that utilises materials, resources, and technology in ways appropriate to the activity planned
Engaging clients:
Care Plan Implementation: Work with individual client and with groups to meet the care, social, leisure/recreational, and accessibility objectives of clients’ care plan
Activity Planning & Execution: Plan and implement centre activities
Client Wellbeing & Safety Management: Take charge of specific health, medical, safety, and hygiene needs of clients
Client Care & Rapport: Build a strong relationship with clients by exhibiting care, concern, and encouragement
Learning Environment:
Holistic & Supportive Learning Environment: Provide physical, emotional and social supportive, and safe environments to enhance motivation, positive behaviour, meaningful engagement, and learning
Behaviour Management: Maintain centre standards and expectations for behaviours
Learning Environment Upkeep: Maintain clean and conducive environment for maximum engagement of clients
Documentation:
- Reporting & Documentation Management: Ensure accurate and timely reporting of programme submissions and other required documentation
Collaboration for Optimal Client Learning & Development
Collaborations with Stakeholders: Work with parents, family, caregivers, community partners, and relevant stakeholders for contextualisation and generalisation of learning at the client’s home and community
Skills & Learning Integration: Work as a team to ensure clients’ integration of skills and learning in activities
Development for Self and Others for Greater Impact
Reflective & Critical Practice: Engage in self-reflective practice and supervision for improved professional practice and delivery.
Process Improvement & Innovation: Participate in initiatives to improve and innovate processes to enhance the care, engagement, learning and development of the clients, and the functioning of the organisation
Job Requirements
Minimum “N” Level / 3 “O” Level passes / partial ”A” Level / fresh NITEC with pass in English
Experience working with clients with disabilities is preferred but not essential.
On-the-job training will be provided.
